Memorandum of Understanding

Below is the Memorandum of Understanding for your youth group outlining the expectations and policies for your campus visit to UW-Madison. Please review all sections and check boxes to indicate your agreement before submitting this tour request form.

The Ingersoll Physics Museum will not assume custodial care of any minor. Visiting groups are responsible for their own minors. Please review the following chaperone/student ratios and chaperone responsibilities:

  1. All teachers, staff, and chaperones have passed a background check by the school district/organization.
  2. All minors have a signed parental permission slip on file with the school district/organization.
  3. All groups must have a minimum of two chaperones and adhere to the following requirements:
    1. 1 adult chaperone for every 10 elementary students (5-11 years of age).
    2. 1 adult chaperone for every 10 middle school/junior high students (11-14 years of age).
    3. 1 adult chaperone for every 18 high school students (14-18 years of age).
  4. Chaperones must take responsibility for students’ behavior on campus. UW-Madison program staff will not act as chaperones for the students.

Ingersoll museum docents are students and may not be available at the requested time. If any one group is larger than 40 and cannot be split, then the tour may end up as a self-guided tour. Please understand that:

  1. The museum is unstaffed and a tour guide may not be available at our requested time.
  2. If a tour guide is unavailable or a group is too large then the group may explore the museum unguided at the reserved time.

The museum is open Monday-Friday from 8:00am to 4:00pm and it is closed on weekends, legal holidays, academic finals weeks, or when the physics department itself is closed. The tour calendar shows any other closure dates and open houses.

If plans change or a group is unable to come for a reserved tour, please contact Cierra Atkinson at cierra.atkinson@wisc.edu as soon as possible to cancel the tour.
